Referrals to Cestreham House will be usually made by individuals needing the service, their families or social workers via a dedicated referral line and/or email address.
Following the referral, the Cestreham House clinical team will conduct a pre admission assessment to establish, firstly that the individual meets the admission criteria and secondly identify the person's care needs so that a care plan can be devised.

A full pre-admisson report together with placement costs will be written by the Cestreham House team and provided to the referrer.
The referrer will, based on the the information and costs provided, either accept or decline the placement. If the placement is agreed, admission will be planned. |
